Eligible to enroll in the JOM Indian Education Program
The 506 form and the student's Tribal Membership card is needed for student eligibility and to generate federal funding for the JOM Indian Education program. However, enrollment in the program does not obligate a student to participate.

The completed 506 form with signature and the image of the student's Tribal membership card can be email directly to Shelly Blackmon at siblackmon@baschools.org by Friday, Sept. 25.
