Oklahoma Caring Van to offer free immunizations to eligible, incoming 7th grade students on May 5

Centennial Middle School will be having an immunization clinic on Friday, May 5, at 9:30 a.m. for middle school students who may need the Tdap immunization. One dose of the Tdap (tetanus, diphtheria and pertussis) vaccine is required for all students entering the 7th grade. Proof of immunization must be submitted to your child’s site before they will be allowed to enter the seventh grade. Students will not be permitted to pick up their schedules without this immunization.

If your child has received the Tdap immunization, please provide documentation to the Centennial's health office.

This clinic will be provided by the Oklahoma Caring Van, and the vaccine will be administered by a registered nurse. Vaccines are offered free of charge to students who are:

  • Sooner Care-eligible
  • Uninsured
  • American Indian, Native Alaskan, Hawaiian or other Pacific Islander

Students will need to present a consent form (Spanish consent form) completed and signed by their parent/guardian to receive the immunization. For questions related to the immunization clinic, please contact Ms. Paul at 918-259-4330 or send her an email.
