Caring Van hosting immunization clinic at BAHS campus, May 5

On May 5, the Caring Van from the Health Department will be coming to the high school to host an immunization clinic for our students who are on Sooner Care, Medicaid, or are American Indian. This is a great opportunity for our seniors who are going off to college to get their meningitis vaccination. The meningitis vaccination is required for college but not for high school. The clinic will also be offering the Tdap and the HPV immunizations to the students. The following pdf links contains more information for parents about pre-teen vaccines:

English pdf | Spanish pdf

The clinic will be held from 9:00 a.m. to 2:00 p.m. in the lobby of the field house (gym). Students can pick up a permission form from the health office, from the attendance office, or print out the following forms.

Page one of the consent form - English | Spanish
Page two of the consent form - Both languages

This form can be returned to the health office before the day of the clinic or can also be brought in with the student the day of the clinic.
