Enroll in Johnson O'Malley (JOM) Services
To be eligible for services through the Johnson O'Malley (JOM) program at Broken Arrow Public Schools, students must be enrolled in the district, and parents must:
- Complete the 506 form
- Send the completed 506 form with signature and an image of the student's Tribal membership card to Shelly Blackmon at siblackmon@baschools.org or by mail to:
