Annual Vision Screening

Broken Arrow Public Schools provides students in Pre-K through eighth grade with a free vision screening.

Vizavance, formerly known as Prevent Blindness Oklahoma, is a nonprofit entity in Oklahoma that supports school districts and parents in providing mandated screening for students. The screeners provided by Vizavance have completed approved vision screening training from the state of Oklahoma, and depending upon the grade level, screening will be conducted using a vision screening camera or a vision chart. Vizavance’s primary goal is to advance children’s education through vision, and by offering vision screenings to Oklahoma’s school children, the organization helps identify vision issues that, if never identified, could seriously hamper a child’s ability to succeed academically. 

If you have questions regarding the screening, please contact Centennial's health office staff at 918-259-4340 ext. 2303, or if you would like for your student to opt out of the screening, please provide a written statement to the health office prior to the screening day. The screening day will be posted under the Centennial's "News" area.


Medication Policy

If your child needs medication during school hours, required paperwork will need to be completed. A copy of the Broken Arrow Medication policy can be obtained through the nurses’ office. “As needed” medications, such as inhalers also require Parent/Physician permission forms. Please remember that no student should be self-administering medication at school without prior approval through the nurses’ office. Any medication sent to school must be brought to the nurses’ office upon the students’ arrival to school. It must be in the original container accompanied by a note from the parent/guardian.

Starting in the school year 2014-2015, over the counter medications (Acetaminophen, Ibuprofen and Antacids) will not be provided by the school sites. Non-prescription medication must be provided by the parent or legal guardian in the original container with the following information: name of the student, printed dosages appropriate for age or weight, and expiration date of the medication. A parent permission form to give medication is also required.


Emergency Health Form

All students must have an Emergency Health Information form on file every year. Please include all pertinent medical information that would be helpful in caring for your child at school. Please remember to include names of any person(s) that you would give permission to pick up your child in the event of illness/injury.


Illness Policy

Students with fever of 100 degrees or higher, vomiting or diarrhea may not return to school until they are fever/symptom free for 24 hours.


Immunization Update

Beginning with the 11-12 school year, incoming 7th grade students are required to show proof of the Tdap Booster prior to schedule pick-up. Whooping cough (Pertussis) is a serious disease that spreads easily through the respiratory route. The incidence of  Pertussis is on the rise in the U.S. Babies and young children are immunized against Pertussis, however the protection typically begins to wear off as children reach their preteen years. Medical experts recommend that preteens get a booster shot called Tdap (Tee-dap) to prevent the Pertussis infection. Other recommended immunizations at this age are a 2nd dose of the Varicella (chicken pox) and the meningitis vaccine. Parents of current 6th grade students, please present documentation of your child’s Tdap vaccine to Nurse Deem as soon as vaccine is administered.
