Homebound/Home Instruction

Students on a 504 Accommodation may receive Homebound services if identified as acutely ill by their physician (DO/MD). The Parent/guardian must provide medical diagnosis that establishes eligibility stating the nature of the health condition and possible length of time the student will need to be away from school. If Homebound services are provided, a school re-entry plan must also be established.  These services are arranged by contacting the student’s school counselor. Each site will have a 504 committee that will address a 504 student's need for services.

Students on an Individual Education Plan (IEP), already actively enrolled in Broken Arrow Public Schools at the time of their illness/injury/medical diagnosis, and who are unable to attend classes due to a medical diagnosis, discipline issues, or programming issues, may be eligible for Home Instruction Services.  The student will be served in a location determined by the IEP team.

A request for Home Instruction (IEP students) may be made when a doctor (MD/DO) has determined that a student will be absent due to accident/injury/ or medical diagnosis.  The parent/guardian must provide medical documentation that establishes eligibility stating the nature of the health condition and the possible length of time the student will need to be away from school to recover form the illness/injury/medical condition.  These services are arranged by notifying the student’s site Counselor or the students file teacher.  The student’s IEP team must meet to place the student on Home Instruction and determine services to be provided.
