Vendor Information
To become a BAPS vendor the following forms will need to be completed and returned to BAPS. These forms are located under "QuickLinks" to your right.
- The New Vendor Request form. The return information is located at the bottom of the form.
- BAPS would also like each vendor to complete the Product and Service form that gives the district a current product list. This will ensure that each vendor is placed in the proper category according to their product line or lines.
- Please complete and return a W-9.
- If doing business on school property please complete the Service Provider Affidavit and return.
