Enrollment Forms
Enrollment Packets are available at the Broken Arrow Public Schools' Enrollment Center located at 210 North Main Street or feel free to download the forms provided below and bring them with you to the Enrollment Center.
Please note that only the parent/legal guardian may enroll the child.
There are a few additional forms that we ask our patrons to complete at our office so we might assist you with any questions you might have.

• Adjudicated Form (Word Document) 1st - 12th Grades ONLY
• H-14 Emergency Information (Word Document)
• Initial Enrollment Prior Participation Form (PDF) Pre-K, Kindergarten, 1st Grade ONLY
